Referral Form

The Referral form is for professionals to refer unpaid/informal Young Carers living in Wrexham, Conwy or Denbighshire

Self Referral Form

This form is for unpaid/informal Young Carers living in Wrexham, Conwy or Denbighshire to join Credu’s mailing list and/or for support. It is a secure link to Credu’s ‘CharityLog’ Database.

By completing this form you give Credu permission to use the information to contact you about your enquiry. You should expect a response within 2 working days.

If you are using this form to make a referral on somebody else’s behalf, you confirm that you have permission from the Carer to share this information with Credu.

WCD takes Young Carers and their family’s privacy seriously and we will use your personal information to enable us to support you and monitor the difference we make.

Personal information may be shared with the relevant local Authority (Wrexham, Conwy or Denbighshire) as a ‘public task’ as part of the Social Services and Wellbeing Act (Wales) 2014. We may suggest other organisations and services that could support Young Carers, but will only pass on personal information to these with Young Carer and parents’ permission, unless someone may be at risk.
